Screening Tool: Lead Poisoning (in Children)

This survey is designed to provide a quick assessment of whether your child has been exposed to lead poisoning. However, no test is 100% accurate. No matter what your score is, you should seek help if you have any concerns about yourself or your loved ones.

 

These questions are based on the Rouke Baby Record screening questions, as recommended by the Centre for Disease Control (CDC).

1. In the past 6 months, has the child lived in a house or apartment built before 1950?
2. Does the child live in a house with recent or ongoing renovations, or peeling or chipped paint?
3. Does the child have a sibling, housemate, or playmate with previous history of lead poisoning?
4. Has the child been seen eating paint chips?
5. Does the child live near lead smelting or processing plants, battery recycling facilities or other sources of lead contamination?
6. Does the child have parents or other household members with lead-related occupations or lead-related hobbies (e.g. pottery or furniture refinishing, practicing on a firing range)?
7. Is the child a refugee aged 6-months to 6-years?
