We provide outreach and support to people in their homes and communities so that they can make the changes that they believe will positively impact their quality of life.
Services include:
Community Support: individualized community-based services to people with persistent mental illness. Intensive case management model whereby a Community Support Worker provides assessment, planning, skills teaching, linking to other services, crisis prevention and advocacy.
Street Outreach: mobile service that assists individuals who are homeless.
Lanauges of service: French; Italian, Hungarian, Farsi, Sinhalese, Swahili, Maltese and Spanish.
